Please fill out this form if you would like to participate in the referral sent to the Canadian Human Rights Tribunal by the Pay Equity Commissioner. To use this form, you must have participated as a party before the Commissioner in the matter to which this referral relates. Refer to the Important Information section (below) and the Canadian Human Rights Tribunal Rules Respecting Pay Equity when completing this form. Please follow the instructions carefully and provide detailed information where requested.
